Referral Process
Eligibility for Admission to a Long Term Care Home
The following criteria must be met to be deemed eligible to enter a long term care home in Ontario:
- The individual must be 18 years of age or older.
- The individual must be a Canadian citizen or a permanent resident.
- The individual must hold a valid Ontario Health Insurance Card.
- The individual must be unable to have their needs met in their existing residence in the community.
The needs of the individual must be able to be accommodated by either the Rekai Centre or Wellesley Central Place.

Process:
- The person seeking admission or their representative must contact the Toronto Community Care Access Centre (CCAC).
- If the case worker from the Toronto CCAC deems the person eligible, the person or their representative is asked to select three long term care homes of their choice.
- During the selection process, the person and/or the representative may wish to tour their potential long term care homes. Most long term care homes offer tours. To tour the Rekai Centre, phone Annie Wong at 416-964-1599 or email annie@rekaicentre.com. To tour Wellesley Central Place, phone Jack MacKeen at 416-929-9385 x 276 or email jack@rekaicentre.com. Annie and Jack are Resident Service Coordinators. They provide specialized support to our residents, coordinate care conferences with the medical team, resident and family. They work hard to facilitate a smooth transition through the admission process.
- When a suitable room becomes available, the person will receive a “bed offer” from the Toronto CCAC.
- Once the person receives a “bed offer” from CCAC, they or their representative have 24 hours to accept the bed offer at which time the Toronto CCAC coordinates the details of their move to the long term care home.
- If the person declines the “bed offer”, they are removed from all waiting lists and not permitted to apply again for a 6 month period.
Please note there are waiting lists at the majority of long term care homes in Toronto.
